2. Print the form out.

3. Complete the registration form. *If paying by credit card please make sure you sign the form and write your name as it appers on your credit card.

4. Clip and mail completed form and your payment to the address provided on the form.

Completed Registration Forms and payments can also be hand delivered to the CCEUC Main Office located at 232 Plaza Road in Kingston during business hours, Monday - Friday 8:30am - 5:00pm.
